The price difference reflects that difference, not just the country you travel to. Knowing which one you are booking matters more than the price tag. Find that out before you sign anything, not after.

A few years ago, a client called me thrilled that she had found a bargain, embryo donation in Spain for EUR 4’000, which sounded impossibly cheap for what she thought she was getting. What she had actually booked was embryo adoption, one single embryo left over from another couple’s IVF cycle. It did not result in a pregnancy. She travelled again, this time to North Cyprus, for what I would call a real donation, a batch created specifically for her from a donated egg and sperm. Every embryo from that donation belonged to her, and she could choose the best one. That pregnancy went to term. It cost more, and it worked.

I have also seen the opposite. A couple who had already spent close to CHF 100’000 in Switzerland got pregnant through an embryo adoption abroad that cost them less than EUR 1’250. Both outcomes are possible. What decides which one you get is not luck, it is understanding exactly what you are paying for before you travel, not after you have already spent the money.

This article focuses specifically on the anonymous versus open question. That is part of the problem. Clinics, agencies and even government websites use the same term for two things that have almost nothing in common once you look at what you actually receive.

Embryo Adoption, Receiving an Already Existing Embryo

Embryo adoption means you receive an embryo that already exists, created during another couple’s own IVF cycle and left over once their family was complete. You usually receive one embryo, sometimes two, from a batch that was never meant for you specifically. Quality varies, and it is not always the best egg quality a full donation would give you, some of these embryos are excellent, others were simply the ones a couple did not use themselves. Waitlists tend to be long, because more people want to receive an embryo than there are embryos available. This is the more affordable route, and for many people it works. It is just not the same product as what most people picture when they hear “embryo donation”.

Phenotype matching is also less reliable with adoption. Because the embryo already exists, you cannot choose or request a donor who resembles you, you receive whichever embryo becomes available. With double donation, clinics can select an egg and sperm donor who match your own features far more closely. For many clients, this matters more than it first appears, they want a child who visually resembles them, so nobody outside the family asks questions about how the pregnancy happened. This is one of the quieter but real reasons some people choose the more expensive route over adoption.

Double Donation, Creating a New Embryo From an Egg and Sperm Donor

Double donation means an embryo is created specifically for you, from a donated egg and donated sperm. Depending on the clinic, you either receive every embryo that results from that donation, including the best one, or the clinic splits embryos from a single donation across several patients. The first version usually costs more and tends to come with higher success rates, because you are not sharing the strongest embryo with someone else.

Why Adoption and Donation Get the Same Name

I have had clients book what they thought was a full donation and receive an adoption instead, at a price that seemed too good to question. Nobody explained the difference beforehand, because in most marketing material there isn’t one to explain. The word “embryo donation” gets used for all of it. That is not dishonest on purpose, the words themselves are simply not used consistently across clinics and countries. But it is exactly the kind of gap that decides whether you come home pregnant or come home and start again.

Open, Semi-Open and Anonymous Embryo Donation Compared

Once you know whether you are looking at adoption or double donation, the next question is what you and your child will ever be allowed to know about the donor. You may receive basic, non-identifying details such as eye colour, height or education, but never a name. This is the model in Spain, the Czech Republic and North Cyprus. It is also the model most people assume applies everywhere, simply because it used to be the default across most of Europe.

Open-ID Donation, Identity Released at Adulthood

Open-ID donation means the donor is identifiable from the start, but only your child, once they reach adulthood, has the right to request that identity. You as the recipient parent typically never learn who the donor is either. Sweden works this way, and so does Finland. Donors in these countries know when they donate that a person they helped create may contact them one day.

Semi-Open Donation, a Term That Means Different Things

Semi-open is the least consistent term in this whole comparison, and clinics use it for two completely different things. Sometimes it means what Portugal does, anonymous now, with the child able to request full identity at eighteen. Other times it means something else entirely, more information than a fully anonymous donation, but never identity, for anyone, ever. That second version can include things like childhood and adult photos of the donor, handwriting samples, or details about siblings, family occupations and hobbies, information that goes well beyond height and eye colour, but still stops short of a name or contact.

Some clinics offer this expanded profile when you specifically ask for it, for example if you mention that sporting ability, musical talent or academic background matters to you, they may show you donor profiles more closely matched to that. They need a cheek swab and a consumer DNA test. A 2020 survey by We Are Donor Conceived found that 78% of respondents who tried had successfully identified their donor through DNA testing, and 70% had located at least one donor sibling this way. None of that required the country’s anonymity laws to change. It simply relies on someone else, a distant cousin, a half-sibling, anyone genetically related who has also tested, being findable in the same database.

This is exactly why ESHRE’s own 2026 position paper pushes for an EU-wide donor registry rather than relying on anonymity as protection. The organisation’s own reasoning is that anonymity as a legal promise and anonymity as a practical reality are no longer the same thing, and clinics that still market “fully anonymous” as a guarantee are describing a legal status, not an outcome they can actually deliver.

What This Means Before You Choose the Anonymous Route

I am not saying anonymous donation is the wrong choice. For many people, it still is the right one, for privacy reasons, for family reasons, for reasons that are entirely their own. What I am saying is that nobody should choose it assuming it is a permanent guarantee. A donor-conceived child with curiosity, a working DNA testing kit, and time on their side has a real chance of finding out on their own, regardless of what the clinic promised at the time. If that possibility changes your decision, it is better to know that before treatment, not after your child mentions they have started looking.

The Batch Question Almost Nobody Asks Before You Choose a Clinic

There is one more distinction that rarely comes up in a clinic consultation, and it has nothing to do with anonymous versus open. The result is genetic half-siblings scattered across different families, who will likely never know the others exist. This is the opposite of what happens with a full-batch donation, where every embryo from one donation goes to a single recipient. It is also why price alone tells you very little. A lower price on a “donation” package can simply mean you are receiving a share of a batch, not the whole batch.

Why a Donor Can Donate Again Elsewhere Without Anyone Knowing

Even where a country sets a legal limit on how many families a single donor can be linked to, that limit is only as good as the system that tracks it. Spain is the one country in this comparison with a national registry, SIRHA, that assigns each donor a single code used across every clinic in the country, specifically so a donor cannot present herself as a first-time donor somewhere else. Most other countries have no equivalent registry. A donor can donate at one clinic, then travel to another clinic, in the same country or a different one, and register as a new donor each time. ESHRE’s 2026 position paper exists precisely because this gap is not a rare edge case, it is the normal situation almost everywhere outside Spain.

None of this means something has gone wrong at your clinic. It means the number of families connected to your donor is, in most countries, simply not something anyone is able to guarantee.

Shipping Your Own Genetic Material Across Borders

Some genetic material and even embryos can travel internationally, separately from you. Sperm banks such as Cryos ship internationally and know exactly which combinations are allowed into which country, an open-identity donor sample cannot legally go everywhere, Spain, for example, does not accept it. I have also seen the reverse case with a client’s own embryos, created in one country, then legally shipped to another for a treatment that wasn’t available where the embryos were originally created. This is its own logistical topic, worth a direct conversation with your clinic before assuming your material can simply move with you.

How to Actually Decide Between Open and Anonymous Donation

There is no universally right answer here, only the right answer for your own family.
