Her role is to prepare her body, usually through hormone medication, so the uterus is ready to receive the embryo.
This distinction matters because it changes what “your” fertility treatment actually involves. You go through monitoring, medication, and the transfer itself. The egg retrieval and the ovarian stimulation happen on the donor’s side, not yours.
Who Considers Egg Donation
People arrive at egg donation for different reasons. Some are told directly that their own eggs are no longer a realistic option. Others have been through several IVF cycles with their own eggs, without a pregnancy, and decide to change approach. Age plays a role for many, since egg quality declines over time in a way that donor eggs bypass entirely. Women who are unable to produce viable eggs of their own, for medical or age-related reasons, make up a large share of who ends up here.
There is no single profile. What most people share is that egg donation was not their first plan on the path to parenthood. It became the option that made sense after looking at the alternatives realistically.

How Egg Donation Works: From Donor Egg to Embryo Transfer
The Donor’s Role and Fertilization
The process starts on the donor’s side, even though this happens outside your own treatment cycle. A donor goes through ovarian stimulation, a period of daily hormone injections that encourage her ovaries to produce multiple eggs in one cycle instead of the usual one. In destinations like Spain and North Cyprus, clinics work with ICSI as standard, where a single sperm is injected directly into the egg, rather than conventional in vitro fertilization. This is done regardless of sperm quality, simply because it is the established approach in these clinics. If donor sperm is needed instead of a partner’s, whether from a sperm bank or from one of the clinic’s own sperm donors, the same fertilization process applies, sometimes combined with intrauterine insemination protocols beforehand to confirm compatibility. Fertilized eggs are then cultured for several days, typically reaching the blastocyst stage before a decision is made about transfer. Not every fertilized egg develops into a usable blastocyst, which is a normal part of the process, not a sign that something went wrong.
Embryo Transfer
The embryo transfer is usually the only procedure you go through directly. A selected embryo is placed inside the uterus through a thin catheter, without anesthesia. It is quick and, for most people, far less demanding than the weeks of preparation leading up to it.
A fresh transfer would require synchronizing your cycle with the donor’s, which is logistically demanding and not always possible, and is one reason fresh donation cycles are rare for international patients specifically. For this reason, most clinics freeze the embryos after fertilization. This allows you to have the transfer in your own natural cycle, at a separate time from the donor’s retrieval, without the pressure of matching two cycles exactly, and it is also simply easier to plan travel around.
Choosing an Egg Donor

Donor Screening and Requirements
Before a donor is accepted into any program, she goes through a screening process: infectious disease testing, a genetic compatibility test to rule out hereditary conditions, and a combined medical and psychological assessment. Most clinics only accept donors within a specific age range, typically between 18 and 32, with the average age of an accepted donor usually in her early twenties, since egg quality is highest in this window. This age range is a large part of why donor pools in places like Spain and North Cyprus lean so heavily on university students. It’s a large, renewable, and naturally young population, which works well for a system that depends on a steady supply of eligible donors. This also means donors must go through the same screening every time they’re matched with a new recipient, not just once at intake.
Donor Profiles and Background
Most donors are matched based on physical characteristics such as hair color, eye color, height, and blood type, aiming for a resemblance to the recipient. Basic health history and genetic testing are standard, though how much of that information is shared with you varies by clinic.
A common assumption is that donors in a given country share that country’s nationality and ethnic background. In practice, this is often not the case. In destinations like Spain or North Cyprus, the donor pool is international, made up largely of young women, including students, from many different countries. North Cyprus in particular has a large international student population, which contributes to a wide and continuously renewed pool of donors. This is part of why waiting times in these locations tend to be short. In many programs, there is no waiting list at all, and a suitable match can be found within the same cycle.
Some clinics work with donor agencies that actively recruit donors more broadly. In some cases, this includes donors who travel specifically for the donation, coordinated through agencies abroad. This adds another layer to how international the donor pool can actually be, beyond what the clinic’s home country would suggest. The number of eggs retrieved and the number of cycles a donor has already completed both factor into how a clinic assesses her ongoing suitability as a donor.
Anonymous and Open Donation
In most destinations for egg donation abroad, including Spain, the Czech Republic, Greece, and Denmark, donation is fully anonymous. As an anonymous donor, she remains untraceable to you: you receive medical and basic physical information about her, but no name, no contact details, and no way to identify her, now or later. Your child will not have a legal path to that information either. This is the default most people encounter, and it is not a lesser option. It is simply how the large majority of international egg donation actually works.
Open egg donation, where identity information becomes available later, does not mean what it sounds like at first: in genuinely open programs, the donor’s identity is not shared with you as the parent. It becomes available to the child, usually between 16 and 18, if the child chooses to request it. Before treatment, in countries that offer open donation, you typically get the same detailed but non-identifying information as with anonymous donation. The difference only appears later. With open donation, your child gains the right, once they reach the legal age in that country, to request the donor’s identity through an official register. You as the parent never receive this yourself.
True open donation, with real donor pools that international patients can access, currently exists in a small number of countries: the UK, Sweden, Finland, France, and Norway. Portugal and Austria sit in between. Donors there are not anonymous, but the child still has no legal right to full identity, only non-identifying information. The Netherlands and Iceland are legally open in a fuller sense, but in practice unusable for most international patients, since clinics there rely on known or directed egg donors, such as a sister or friend, rather than donor pools, and importing eggs is not permitted.
Some clinics elsewhere also work with frozen donor eggs from an egg bank rather than a fresh, synchronized donation, which can shorten waiting times further, though this is less common than fresh donor-egg cycles. This known-donor model, using a family member or friend as the donor, is one reason true open donation stays rare in Europe: most countries either don’t allow it, or only allow it in this very limited, personal form. Egg donation itself remains prohibited in Switzerland, Germany, Turkey, and Bosnia and Herzegovina.
Officially, adult photos of the donor are not part of any open-donation program, even in the UK or Scandinavia. In practice, some agencies and individual clinics work around this. I know of agencies that share childhood and later adult photos of a donor outside the formal open-donation framework, and at least one clinic in North Cyprus that provides donor photos for an additional fee. There are also agencies, including one I know of in South Africa, that place open, identifiable donors specifically in countries where directed donation is legally accepted, such as North Cyprus, though this depends entirely on the individual clinic and is not available everywhere. Even where open donation is possible, the number of donors willing to be identifiable later is small, which creates a mismatch between what people want and what is realistically available.
How Much Choice You Actually Have
The level of detail you receive about a donor differs significantly between clinics. Some offer only basic physical matching with no further input from you. Others allow more specific preferences, sometimes described as choosing based on interests or background, usually available for an additional cost. Some clinics in Spain use software like Fenomatch, which analyzes facial features to find the ideal donor match for you. Others rely on a committee of staff who discuss and agree on a suitable donor internally.
One detail that surprises many people: a single donor’s eggs are usually not reserved exclusively for you. Most programs distribute the eggs from one donation cycle across several recipients, which keeps costs lower than a fully exclusive donation would, but also means you’re not the only one matched with that particular donor. If having a donor exclusively to yourself matters to you, that’s worth asking about directly, since it usually comes at a higher price. It’s also worth asking a clinic how many other donor and recipient pairings might be matched with the same donor in a given cycle, since this affects both cost and, in some cases, waiting times.
Some clinics also let you review a short list of donors who match your basic criteria before confirming, rather than assigning one directly, though this varies by clinic and is not the norm everywhere.

Shared Motherhood: A Different Kind of Egg Donation
How It Differs from Standard Egg Donation
The medical process looks similar to standard egg donation on paper: one person undergoes ovarian stimulation and egg retrieval, the eggs are fertilized, and an embryo is transferred into the other partner’s uterus. The difference is that the “donor” here is not an anonymous or open third-party donor, but the intended mother’s own partner. There is no donor selection process, no matching, and none of the anonymity questions that apply to standard donation.
Depending on the country and clinic, either partner may be medically suitable to stimulate and provide eggs, and either may be suitable to carry the pregnancy. This is not automatically decided by preference. It usually comes down to a medical assessment of who is more likely to produce good egg quality and who is more likely to carry the pregnancy well, and couples often need to weigh both factors together. Some countries require the couple to be married or in a registered partnership before treatment, which is worth checking early, since it can affect whether a clinic is able to proceed at all.
Where Shared Motherhood Is Possible
Egg donation being legal and lesbian motherhood being recognized do not always go together, which makes this more complicated than it first appears. ROPA is available in Spain, the UK, Portugal, Belgium, and the Netherlands, with some restrictions in Greece. It is not permitted in Switzerland, Germany, Italy, the Czech Republic, Poland, France, Denmark, Norway, Sweden, or Finland, even in countries where standard egg donation itself is allowed.
Austria is a specific exception. ROPA itself is not available there, but sperm donation for female couples is, and Austria has a legal advantage most other countries lack: both mothers are recognized as legal parents from birth. Everywhere else that permits shared motherhood, the non-biological or non-birth mother typically has to apply for stepchild adoption after the birth to be legally recognized, which is a separate legal process most couples do not expect to need.
In North Cyprus, ROPA is only offered by a small number of clinics, and I currently know of only one clinic there that is genuinely LGBT+ friendly in how it works with same-sex couples.

Initial Screening and Preparation
Before treatment starts, clinics require a more thorough round of testing than most people expect. This typically includes bloodwork for infectious diseases such as HIV, hepatitis B and C, syphilis, and rubella immunity, along with a transvaginal ultrasound to check the uterus and rule out issues like fibroids, cysts, or polyps that could affect implantation. Your gynecological and health history, any previous surgeries on the uterus or ovaries, and any regular medication are usually reviewed as part of this too. If a partner’s sperm is being used, he typically goes through a similar infectious disease panel and a semen analysis, though this is a smaller part of the process compared to what you go through yourself.
Much of this testing can be done at home through your own doctor or a local lab, with results sent to the clinic in advance. Doing this before you travel is what allows the treatment itself, once you arrive, to be organized within a shorter, more predictable window.
Your Cycle, Step by Step
Once cleared, you start a course of estrogen to prepare and build up the uterine lining, timed around the donor’s cycle or around using frozen embryos, depending on which approach the clinic uses. For most women, this takes about 10 to 14 days, with monitoring through ultrasound to track how the lining develops. If the lining responds more slowly, the clinic may extend this to three or four weeks, occasionally up to six. Longer than that is uncommon, and usually only happens where there is an underlying issue with the lining itself, not because of age or menopausal status.
A question that comes up often: does being in menopause change any of this? In practice, it doesn’t make preparation longer or more complicated. Without your own natural hormone cycle, the clinic controls the timing directly with medication, which if anything makes planning more predictable, not less.
The uterus itself stays responsive to hormone preparation well beyond the point where natural fertility ends, which is part of why egg donation works for women well into their fifties.
On the donor’s side, daily injections of follicle stimulating hormone, or FSH, drive the ovarian stimulation itself, alongside other hormonal medication that controls timing. Fertility drugs used during stimulation carry some health risks for the donor, most notably ovarian hyperstimulation syndrome, which is why reputable clinics monitor donors closely throughout this phase.
The clinic checks lining thickness, aiming for roughly 7 to 9 millimeters, along with its structure, before deciding it is time to start progesterone and schedule the transfer, usually about five days after progesterone begins for a blastocyst-stage embryo. Estrogen and progesterone together are what prepare the endometrium to receive the embryo. This is also the point in the cycle where, for a fresh transfer, your cycle and the donor’s need to run on the same rhythm, usually managed with birth control pills that delay or shift your period so it lines up with hers. This synchronization is demanding, since it depends on two people’s bodies responding to medication in a coordinated way, which is a large part of why many clinics prefer working with frozen embryos instead.
How Many Trips You Actually Need
With a frozen embryo transfer, the process usually splits into two separate trips rather than one longer stay. If a partner’s sperm is used, he typically travels first, for a single day, to provide the sample, which is then used for fertilization alongside the donor’s eggs. Weeks later, once the embryo has developed and any genetic testing is complete, you travel separately, again usually for just one day, for the transfer itself. Neither trip requires more than a short stay, and the two don’t need to happen close together.
Travelling together as a couple is also possible, but it means aiming for a fresh transfer instead, with the cycle synchronization described above. This is more demanding to coordinate and leaves less room for delays, which is part of why many couples choose to split the trips instead, even if it means travelling separately.
Because the donor’s side of the process runs somewhat independently from yours, exact dates often aren’t fixed until fairly close to the actual retrieval. Clinics generally give you a window rather than a precise date early on, which makes it worth keeping some flexibility around work or family commitments during the weeks a cycle is active. Some people prefer to keep treatment private. Doing the entire process abroad, with short, separate trips rather than a long stay, naturally supports that, since there’s no extended absence to explain.
Age Limits by Country
Most countries that allow egg donation set an upper age limit for treatment, though it varies. Fifty is the most common cutoff. The Czech Republic is stricter, at 48. Spain also generally applies 50, though some clinics make exceptions and transfer beyond that on a case-by-case basis. North Cyprus tends to be the most flexible, with treatment sometimes available up to around 55. If your age is close to a country’s limit, it is worth confirming directly with a clinic rather than assuming a general rule applies everywhere.
Legal Aspects of Egg Donation
Unlike some other treatments covered on this site, egg donation is a clearly regulated area in most countries, not a legal grey zone. That makes it possible to talk about legality directly here, rather than working around the word. Travelling abroad for treatment that isn’t permitted at home is not illegal. You are not breaking any law by receiving egg donation in a country where it is legal, even if it isn’t legal where you live.
Where Egg Donation Is Not Permitted, and Why
Egg donation remains prohibited in Switzerland, Germany, and Bosnia and Herzegovina. Turkey is a specific case: egg donation is prohibited for Turkish citizens, but this restriction doesn’t apply to international patients travelling to North Cyprus, which operates under its own legal framework.
In Switzerland and Germany, the restriction goes back to laws written in the 1990s, built around the idea that splitting motherhood into a genetic and a gestational part, one woman providing the egg, another carrying the pregnancy, creates legal complications the law wasn’t willing to allow at the time. The concern was less about the medical procedure itself and more about protecting the child’s clear, single legal parentage, and about preventing a market where eggs could be bought and sold. That reasoning hasn’t really been revisited since, even though sperm donation, which raises similar questions about genetic versus legal parentage, has been legal in both countries for far longer.
Changing this kind of law is slow, even once the reasoning behind it is questioned. Switzerland is currently reviewing its position, and legalization is considered realistic within the next few years, though it requires parliamentary process and hasn’t happened yet. Even once it passes, actual availability, in terms of donors willing to participate, will likely take longer still to catch up with legal permission. In the meantime, people in these countries aren’t waiting. Egg donation abroad has been a well-established, unremarkable option for years, regardless of where the law currently stands. Legal Parentage
Who counts as the legal mother matters just as much as who is medically involved. With standard egg donation, the woman who gives birth is legally recognized as the mother in virtually every country, regardless of whose egg was used. This part is usually straightforward.
It gets more complex with shared motherhood. As covered earlier, Austria is the only country where both partners are recognized as legal parents from birth. Everywhere else that allows ROPA, the partner who didn’t carry the pregnancy has to apply for stepchild adoption afterward to be legally recognized, a process that takes time and isn’t automatic.
Why People Travel Even Where It’s Legal
Even in countries where egg donation is legal, plenty of people still travel abroad for it. Cost is one reason, since prices vary significantly between countries and out-of-pocket treatment can be considerably cheaper elsewhere. Access to preimplantation genetic testing, gender information, or a choice between anonymous and open donation are others, since these options are not available everywhere egg donation itself is legal. Waiting times and donor availability also play a role, along with differences in clinical practice between countries. This is part of why British patients often travel to Greece, and why “legal at home” doesn’t always mean “the best option at home.”

Whether embryo freezing beyond the first year, or storing embryos for future use, is included or billed separately also depends on the clinic.
The number of guaranteed blastocysts is one of the biggest price drivers, and it’s also why giving a single, exact price is difficult. A cycle with no guarantee is cheaper upfront, but carries more risk if it doesn’t produce a usable embryo. A cycle with a higher guarantee costs more from the start, but reduces the chance of needing to pay for a second attempt. As mentioned earlier, most programs also share one donor’s eggs across several recipients rather than reserving them exclusively, which keeps the base price lower than fully exclusive donation.
Why Prices Differ Between Countries
Prices vary significantly between countries, partly due to differences in healthcare costs, donor compensation, and regulation, and partly due to how much competition exists between clinics locally. Spain and North Cyprus are both established destinations with a high volume of international patients, but North Cyprus is generally the more affordable of the two, largely because of lower operating costs and a larger, faster-moving donor pool. Where open donation is possible at all, prices tend to be considerably higher than for anonymous donation, since the donor pool willing to be identifiable is much smaller relative to demand.
The lowest price is not automatically the best value, and a higher price doesn’t guarantee a better outcome either. Success rates with donor eggs are generally higher than with a woman’s own eggs at an advanced age, since the eggs come from young, healthy donors rather than being affected by the recipient’s age. In North Cyprus, success rates typically range between 70 and 80 percent per transfer. Miscarriage rates are correspondingly lower than for an older woman using her own eggs, usually around 10 to 15 percent, closer to what a younger woman would expect. With three euploid embryos tested through PGT-A, cumulative success rates can reach 90 to 95 percent. This is one of the clearest practical reasons donor eggs are used to achieve pregnancy where a woman’s own eggs no longer offer realistic odds.
After a Positive Result
A positive blood test isn’t the final confirmation. Clinics typically schedule an ultrasound around two weeks later to confirm a heartbeat and that the pregnancy is developing correctly inside the uterus. This is also the point where your doctor needs to know the pregnancy came from egg donation, since it changes how certain risks should be assessed compared to a pregnancy with your own eggs at the same age.
If the Result Is Negative
Not every cycle leads to a pregnancy, even with a donor egg. A rare but serious complication to be aware of, mainly relevant on the donor’s side during stimulation, is ovarian hyperstimulation syndrome, or OHSS. In critical OHSS cases, which are rare, clinics may need to postpone a fresh transfer entirely and rely on frozen embryos instead, thawed only once the donor has recovered. On the recipient’s side, if the test is negative, the next step depends on what’s included in your treatment package: whether there’s a frozen embryo remaining for another attempt, or whether a new donor cycle is needed. This is exactly why it’s worth clarifying this beforehand, rather than finding out only once a cycle doesn’t work.
